- been out of state and NOT complied with the testing/quarantine requirements in the state travel advisory.
–OR–
Within the last 48 hours:
- been experiencing COVID related symptoms as outlined by the CDC.
You AGREE to:
- Wash hands prior to massage.
- Wear a mask while the massage therapist is in your home.
